At Facet and Form, your safety and comfort are our highest priorities. To comply with Washington State law and maintain a secure environment for all of our clients, we enforce strict identification policies for body piercing.
Please review our ID requirements below before arriving for your appointment. If you arrive without the proper documentation, we will be unable to perform your piercing and you may forfeit your appointment deposit.
Adult Clients (18 and Older)
If you are 18 or older, you must present a current, physical, goverment-issued photo ID to receive a piercing.
What We Accept:
- State-issued Driver's License or ID Card
- Canadian Driver's License or ID Card
- US or Foreign Passport
- Tribal ID
What We DO NOT Accept:
- Military IDs: Please note that federal law (18 U.S. Code § 701) makes it illegal for commercial businesses to photocopy military IDs. Because we are required to keep a copy of your ID on file with your consent form, we cannot accept them.
- Photocopies of IDs (paper copies)
- Digital photos of an ID on your phone
- Expired IDs (no exceptions)
- School or University IDs
- Birth certificates alone (for adults)
MINOR CLIENTS (UNDER 18)
Washington State law requires any minor receiving a piercing to have a parent or state-appointed legal guardian present for the entirety of the procedure to provide written, informed consent. We require documentation for both the minor and the parent/guardian to prove identity and relationship.
1. Identification for the Parent/Legal Guardian
The accompanying parent or legal guardian must present a current, physical, government-issued photo ID (see the Adult accepted list above).
2. Identification for the Minor
- Ages 13–17: A valid photo ID (School ID, State ID, Learner's Permit, or Passport) AND an original or certified copy of their Birth Certificate.
- Ages 12 and Under: An original or certified copy of their Birth Certificate. (Note: Please contact the studio directly for age restrictions on specific piercings).
3. Proof of Relationship
The documents provided must reasonably establish that the accompanying adult is the legal parent or guardian.
- If your last names match: The minor's Birth Certificate with the parent's name on it is sufficient.
- If your last names do NOT match: You must provide legal documentation linking you. This can include:
Marriage certificates (if a parent's name has changed since birth)
Divorce decrees showing a legal name change
State-issued legal guardianship papers (signed by a judge)
Adoption papers.
What We Do NOT Accept for Minors:
- Non-Legal Guardians: Older siblings, aunts, uncles, grandparents, or step-parents cannot sign for a minor unless they have court-ordered legal custody documents. A hand-written note or phone call from a parent is not legally valid.
- Digital Copies: Photocopies or cell phone pictures of any documents (IDs, birth certificates, or court papers) are never accepted. All documents must be originals.
- Hospital Certificates: Hospital-issued "footprint" birth certificates are not valid; we require the state or county-issued certified copy.
